Liability Waver




I understand and acknowledge that participation in physical exercise, training programs and the use of gym equipment at Body Pulse by Lynroy involves inherent risks, including but not limited to injury, muscle strain, falls, equipment malfunction, or medical complications. I voluntarily choose to participate in activities at this facility.
I certify that the information I have provided regarding my medical history is accurate to the best of my knowledge. I understand that it is my responsibility to consult with a physician prior to engaging in any fitness program.

By signing this form, I:

  • Assume full responsibility for any risks, injuries, or damages that may occur.
  • Release and discharge Lynroy Henry, all trainers, all employees, and representatives from any and all liability, claims, demands, or causes of action resulting from my participation.
  • Agree to follow all gym rules, safety guidelines, and instructions provided by gym staff. 
  • Acknowledged that this waiver is legally binding to the fullest extent permitted by law.
